The PSS-10 produces a raw score from 0 to 40. Scores of 0–13 indicate low perceived stress, 14–26 moderate, and 27–40 high perceived stress. These are the conventional reporting bands used in stress research.

How this is scored

A validated instrument, scored exactly as published.

The 0–40 number is not ours. It is the Perceived Stress Scale, introduced by Cohen, Kamarck and Mermelstein in 1983 and still the most widely used psychological instrument for measuring the perception of stress. We score it to the letter and only then add our own overlay on top — clearly separated.

01 · Instrument

Ten items, five-point scale

Each item is rated 0 (never) to 4 (very often) for the last month. Items 4, 5, 7 and 8 are worded positively and are reverse scored (0→4, 1→3, and so on) before summing. The total runs 0–40. Internal consistency across validation samples is consistently good, with Cronbach's α above 0.78.

02 · Bands

Established cut-points

0–13 low perceived stress, 14–26 moderate, 27–40 high. These are the conventional reporting bands in the PSS literature, not thresholds we invented. US normative means sit in the low-to-mid teens, so a score around 13–14 is close to average, not exceptional.

03 · Overlay

Resilience age, kept separate

The resilience age is our own transparent overlay — a readable translation, not a validated biomarker. It starts at your chronological age and shifts it using the PSS score, two recovery questions, and (optionally) your resting HRV against the population curve for your age.

The HRV curve is fitted to commonly reported short-recording RMSSD norms, which fall from roughly 60 ms in the early twenties to the low twenties by age seventy. HRV is highly individual: the same reading can be healthy in one person and low in another, which is why it is blended rather than used alone.

Stress & aging

What chronic stress actually does to the body.

The acute stress response is not the problem — it is fast, useful, and self-limiting. The damage comes from a response that never fully switches off. Three mechanisms carry most of that cost.

Cortisol

A rhythm that flattens

Healthy cortisol has a steep daily curve: a sharp rise 30 minutes after waking, then a long decline into the evening. Under sustained stress that slope flattens — lower peaks, higher nights. A flattened diurnal slope has been associated with worse outcomes across a range of conditions, and it is one reason chronic stress and poor sleep are so hard to separate.

Inflammation

Low-grade, systemic, persistent

Prolonged stress shifts immune signalling toward a pro-inflammatory profile, with elevated IL-6 and CRP. This is the common pathway linking psychological stress to cardiovascular disease, metabolic dysfunction and depression — the same low-grade inflammation that features in most models of biological aging.

Allostatic load

The bill for staying switched on

McEwen's framing: the systems that protect you in the short term wear you down when they never stand down. Allostatic load is the cumulative cost across cardiovascular, metabolic, immune and neural systems — and it tracks the duration of the response far more closely than its peak.

Where HRV fits

Heart-rate variability is the beat-to-beat variation in the interval between heartbeats. A healthy heart is not a metronome: under parasympathetic (vagal) influence, the interval flexes constantly. Higher resting variability generally indicates a nervous system that can shift gears — up for demand, down for recovery.

Across research, higher HRV is associated with better self-reported sleep, lower fatigue and lower perceived stress, which is why it makes a useful cross-check on a self-report questionnaire. It is not a substitute for one. HRV varies enormously between individuals — by age, fitness, genetics, posture and measurement method — so the meaningful signal is your own trend over weeks, not your number against someone else's.
